Surgical Extraction of Wisdom Teeth – Alveotomy
Wisdom teeth or third molars are permanent teeth that erupt last, though not everyone has them. Some people have normal, painless wisdom tooth eruption, while others experience serious problems, pain, inflammation, and shifting of other teeth. Sometimes, there isn’t enough space in the jaw for them to emerge, and they may grow in the wrong direction. In all these cases, surgical extraction of the wisdom tooth is necessary.

Wisdom tooth extraction is recommended in cases of:

Complications related to orthodontic treatment to straighten other teeth
Wisdom tooth extraction is also advised when there isn’t enough space for the tooth to grow, causing pain, infections, or other problems. Often, wisdom teeth are removed on the recommendation of an orthodontic specialist, even if the above symptoms and problems are not present in the oral cavity. Extraction of impacted wisdom teeth (those not fully erupted) may require a more complex surgical approach involving incision of the gums and removal of a bone fragment.

How the Wisdom Tooth Extraction Procedure Works
Examination and Preparation
Before the procedure, a clinical and radiographic examination is performed, followed by a consultation with the oral surgeon to discuss your overall health, ask questions, and choose the type of anesthesia.
Choice of Anesthesia
The procedure is performed under local anesthesia to numb the affected area. If you experience anxiety or fear, conscious sedation is available with the assistance of an anesthesiology specialist.

The wisdom tooth extraction is painless and takes about 30 minutes.
Rest and Instructions for Optimal Recovery
After the procedure, a rest period of at least 30 minutes is scheduled in the relaxation room. Detailed instructions on postoperative care and wound management are provided.
Surgical Removal of the Root Apex – Apicoectomy
More details on the procedure
Apicoectomy is the ideal solution if the tooth is well preserved and the inflammatory process has not severely damaged the healthy bone above the tooth. This minor surgical procedure is also performed when the root canals are excessively curved or bent and cannot be properly cleaned with endodontic therapy.
The Procedure
The procedure is performed under local anesthesia to ensure it is painless.
The gums are incised to allow the oral surgeon to access the root. Our oral surgeon performs the procedure using micropiezo devices equipped with a special curved-tip insert, which facilitates the procedure even in cases of hard-to-reach roots.
Depending on the tooth being treated, the apicoectomy can last from 30 to 60 minutes. The procedure is shorter for anterior teeth and longer for posterior teeth. The apicoectomy is successfully completed in a single session, and a follow-up visit to remove the sutures is scheduled after one week. If the patient cannot attend the suture removal appointment, absorbable sutures can be used, which will dissolve on their own within three weeks.
After six months, a follow-up visit and an intraoral dental X-ray are necessary.

Sinus Lift Procedure
Long-term absence of one or more teeth in the posterior oral cavity region can lead to progressive loss of jawbone. Without enough bone to support them, the sinuses expand and occupy the space previously filled by bone. In this case, to successfully place implants and restore chewing function and tooth aesthetics, a sinus lift is necessary to reposition the sinuses. It may sound complicated, but at Orto Nova, the sinus lift is a completely painless and routine procedure performed by our oral surgery specialist.
Depending on the amount of available bone, the sinus lift procedure is divided into:
Small Sinus Lift
We perform the small sinus lift simultaneously with implant placement in patients who have sufficient bone volume. Using a small instrument, the surgeon lifts the sinus floor back to its original position and then inserts artificial bone graft material along the existing bone beneath the elevated sinus. The artificial bone stimulates new bone formation and maintains the sinus at the necessary height.
Large Sinus Lift
We perform the large sinus lift in patients who do not have sufficient bone volume. This procedure is done through a lateral access to the oral cavity. For patients with significant bone atrophy, the sinus lift must be performed first, followed by implant therapy. Implants can be placed about eight months after this procedure, once ossification is complete. The goal of the procedure is to fill part of the sinus cavity with a biocompatible material that will ossify and create a solid base for future implants.

Cystectomy
A cystectomy is an oral surgery procedure to remove a cyst. The cyst is often visible on the patient’s panoramic X-ray, while visible changes to the naked eye are usually concentrated around the tooth root.
The most common symptoms are:
Discoloration of the tooth
Extensive decay
Bump on the bone (jawbone) above the teeth
Fistula
Pain, swelling, or redness around the tooth

Procedure and Recovery
To make the procedure painless, we apply local anesthesia.
The oral surgeon removes the cyst, shortens the root apex, and seals the tooth canals.
The cavity left after cyst removal can be filled with bone graft material to promote efficient healing. The incision is closed and sutures are applied.
The sutures are removed one week after the procedure.
